Our Path to Purpose

Upstate Holistic Wellness grew from our shared belief that physical health, emotional well-being, relationships, stress, and personal stories are deeply connected.

Together, we have created a place where functional medicine and trauma-informed therapy can work from the same map, honoring both the body and the person living within it. Our hope is to offer thoughtful, whole-person care that helps people understand their health, feel supported in the healing process, and make meaningful changes that can be sustained within real life.

  • Isabelle transformed My
vision into reality. The 72% growth exceeded every expectation.

The Art of Working
Together

Jake Duncan, PA-C

Jake Duncan is a board-certified physician assistant with advanced training in functional medicine. His approach was shaped by his health journey, including two heart surgeries and rebuilding his strength and resilience.

Jake works with people whose symptoms feel layered, confusing, or difficult to untangle. He considers the relationships among gut health, hormones, thyroid function, metabolism, inflammation, immune patterns, nutrient status, sleep, stress, movement, and emotional health. Through careful listening, lab testing, education, and individualized protocols, he helps people understand what may be contributing to their symptoms. Jake believes people heal best when they understand their bodies and can participate in their care. His goal is to create a clear, holistic, practical plan that fits each person’s body and everyday life.
